Every Student Must Fill Out a Waiver or Enrollment Form Every Year:
State Mandatory Health Insurance Requirement

Visiting www.gallagherkoster.com will take you to the Koster Insurance web site for completion of waiver or enrollment form. Select "Student Access" and choose Western New England College from the drop down box. All waivers or enrollments must be completed online by September 18th to remove the health insurance charge of $1780 from the student tuition bill.

 

Click on, print, complete and submit by August 1st the following forms that apply to you:
Freshman or Transfer Student
Form #2
· This form must be completed by a health care provider and you.

Freshman or Transfer Athletes
(not intramurals)
Form #2, Form #3
· These forms must be completed by a health care provider and you.
· Physicals and Pre-participation Evaluation forms must be completed/dated
  no longer than 6 months prior to the start of participation.
Full Time Law Student
Form #4
· This form must be completed by a health care provider and you.

Returning Football Player
Form #3, Form #5
· These forms must be completed by a health care provider and you.
· The forms must be completed/dated no longer than 6 months prior
  to the start of participation.
